Study: Drinking coffee prevents development of Alzheimer’s, Parkinson’s

A massive new study followed more than 130,000 people for up to 43 years.

The finding: people who drank 2 to 3 cups of caffeinated coffee a day had an 18% lower risk of developing dementia than those who rarely touched the stuff.

That’s not a small effect.

It comes from researchers at Mass General Brigham, Harvard’s T.H. Chan School of Public Health, and the Broad Institute, published in the journal JAMA.

They tracked participants from the long-running Nurses’ Health Study and Health Professionals Follow-Up Study.

Over four decades, 11,033 people in the study developed dementia.

But the ones who drank moderate amounts of caffeinated coffee weren’t just less likely to be diagnosed.

They also reported fewer memory complaints and performed better on cognitive tests.

So if you’re sipping your second cup right now, you can relax a little.

But before you assume more is always better, there’s a twist coming.

And it changes the whole conversation about coffee and brain health.

The Real Headline Isn’t “Coffee Prevents Disease”

Here’s where most coverage of this topic gets it wrong.

Coffee does not “prevent” Alzheimer’s or Parkinson’s in the way a vaccine prevents measles.

What the research actually shows is an association, a statistical pattern linking habits to outcomes over time.

That distinction matters more than it sounds.

An association means people who drink moderate coffee tend to have lower rates of disease.

It does not mean coffee is a guaranteed shield, or that skipping coffee dooms you.

Dr. Daniel Wang, the senior author of the Harvard-led study, was careful about this.

He noted that the effect size is small and there are many important ways to protect cognitive function as people age.

In other words, coffee is one piece of a much bigger puzzle, not a silver bullet.

That’s the honest, less catchy version of the story.

And it’s actually more useful than the headline version.

But Here’s What Most People Get Wrong About “More Coffee, More Protection”

This is where the story gets interesting.

Most people assume that if a little coffee helps, a lot of coffee must help even more.

That assumption is wrong, and a separate large study proves it.

Researchers analyzed health data from over 134,000 people in the UK Biobank, following them for a median of 13.5 years.

They found something unexpected: a clear J-shaped relationship between coffee intake and neurodegenerative disease risk, according to the study published in Nutrition Journal.

That J-shape is the twist.

Risk drops as coffee intake increases from zero to a moderate level.

But once intake climbs past roughly 4 to 5 cups a day, risk starts climbing again.

People who drank more than five cups daily had a higher risk of developing primary neurodegenerative diseases, including Alzheimer’s, compared with people who drank none at all.

That’s the opposite of what the simple “coffee is good for your brain” headline implies.

Coffee isn’t a dial you can turn up indefinitely for more benefit.

It behaves more like a window. There’s a sweet spot, and stepping outside it in either direction changes the outlook.

Why Heavy Coffee Drinking Might Backfire

The researchers behind the UK Biobank study offered a few possible explanations.

One is biological. Heavy coffee intake has been linked to increases in serum cholesterol and triglyceride levels, both of which raise the risk of vascular damage in the brain.

Another involves brain volume.

A controlled trial cited in the study found that participants who consumed caffeine daily for a week showed a measurable decrease in gray matter volume compared with a placebo group.

A separate UK Biobank analysis found something similar: heavy coffee drinkers tended to have smaller total brain and gray matter volumes over time.

None of this means an occasional fourth or fifth cup will damage your brain.

It means habitual, heavy intake, day after day, year after year, is a different story than a daily ritual of two or three cups.

How the Study Was Conducted

It’s worth understanding how researchers actually reached these conclusions, because the methodology is part of why this research carries weight.

The Harvard-led study pulled from two of the most respected long-term health databases in existence.

Participants self-reported their coffee, tea, and decaf intake repeatedly over decades, alongside detailed health and lifestyle data.

Researchers then tracked who developed dementia, who reported cognitive decline, and who performed worse on memory and thinking tests.

The UK Biobank researchers took a similar but distinct approach.

They used a statistical method called a restricted cubic spline, which is designed specifically to detect non-straight-line, or “nonlinear,” relationships in data.

This is the technique that revealed the J-shaped curve.

A simpler analysis might have missed it entirely and just reported a flat “coffee is good” or “coffee is bad” conclusion.

They also adjusted for dozens of other factors, including smoking, drinking, sleep, blood pressure, education, income, and even specific genetic variants that affect how fast someone metabolizes caffeine.

That last detail matters because some people process caffeine quickly and others slowly, based on a gene called CYP1A2.

Interestingly, the researchers found that this genetic difference didn’t change the overall pattern. Fast and slow metabolizers showed similar associations between coffee and disease risk.

Findings From the Study

Putting the two major studies together paints a clearer picture than either one alone.

Moderate caffeinated coffee intake, around 2 to 3 cups daily, is linked to meaningfully lower dementia risk.

The Harvard-led research found an 18% reduction in dementia risk among regular caffeinated coffee drinkers.

Decaffeinated coffee did not show the same protective association.

This is one of the more striking details. Decaffeinated coffee did not show the same associations as caffeinated coffee or tea, suggesting that caffeine itself, not just coffee’s other compounds, plays a central role.

Tea showed similar benefits to coffee.

People who drank 1 to 2 cups of tea daily also saw reduced dementia risk in the Harvard study, and the UK Biobank research found tea intake was tied to lower risk across several specific conditions, including Parkinson’s disease.

Genetic risk didn’t cancel out the benefit.

Lead researcher Yu Zhang explained that coffee or caffeine appeared to be similarly beneficial for people with both high and low genetic risk for dementia.

That’s reassuring for anyone who has a family history of Alzheimer’s and wonders whether lifestyle choices even matter.

But heavy intake flips the script.

More than five cups a day of coffee was associated with a higher risk of primary neurodegenerative disease, including a slightly elevated risk for Alzheimer’s specifically, according to the UK Biobank data.

Coffee type might matter too.

The same study found that ground coffee, rich in a compound called chlorogenic acid, was linked to lower risk, while the picture for instant and decaf varieties looked different.

Parkinson’s disease showed a more complex pattern.

Coffee itself didn’t show a strong, statistically clear link to Parkinson’s risk in the UK Biobank data, but tea intake did. People drinking more than three cups of tea daily had a notably lower risk of developing Parkinson’s.

This nuance rarely makes it into headlines, but it’s an important part of the real story.

How the Study Applies to Real Life

So what does someone actually do with all of this?

The practical takeaway is refreshingly simple, even if the science behind it is complex.

Two to three cups of regular, caffeinated coffee a day appears to sit in the sweet spot for potential brain benefits, based on current evidence.

That’s roughly what a lot of people already drink without thinking twice about it.

If you’re someone who drinks five, six, or more cups daily, this research is a gentle nudge to consider scaling back, not because one extra cup will cause harm, but because the long-term pattern suggests diminishing or even reversing returns.

If you don’t drink coffee at all, this isn’t a reason to start. Tea offers a similar potential benefit, and plenty of other lifestyle factors, like sleep, exercise, and social connection, play at least as large a role in brain health.

Context also matters.

The UK Biobank study found that people with hypertension who drank more than five cups of coffee daily had a meaningfully higher risk of neurodegenerative disease than non-drinkers with hypertension.

That’s a useful reminder that general findings don’t always apply identically to every individual, especially those managing existing health conditions.

If you have high blood pressure, heart issues, or anxiety that’s worsened by caffeine, the calculus changes, and a conversation with a doctor is more useful than a public health headline.

The Bigger Picture Behind the Cup

It’s tempting to treat a single beverage as either a hero or a villain.

Real biology rarely works that way.

Coffee contains caffeine, yes, but also polyphenols with anti-inflammatory and antioxidant properties that may help protect neurons from the kind of damage linked to Alzheimer’s and Parkinson’s.

At low to moderate doses, those compounds appear to support brain resilience.

At very high doses, other effects, including changes in cholesterol, blood pressure, and even brain volume, may start to outweigh the benefits.

That’s not a contradiction. It’s just how dose-dependent biology tends to work, similar to how exercise, sunlight, and even water follow a “more isn’t always better” curve.

A Few Honest Limitations Worth Knowing

No single study, however large, settles a question this complex.

Both the Harvard-led research and the UK Biobank study are observational, meaning they tracked what people already chose to drink rather than randomly assigning coffee or no coffee to different groups.

That means researchers can identify strong patterns, but they can’t fully rule out that something else about heavy or light coffee drinkers, like overall diet, stress levels, or other habits, is influencing the results.

A related Mendelian randomization study, which uses genetic variants to test cause and effect more rigorously, found that higher genetically predicted coffee consumption was linked to a higher risk of Alzheimer’s, adding a layer of complexity rather than a clean resolution.

Science rarely hands over a tidy, final answer. It hands over better and better approximations the more evidence accumulates.

Right now, the evidence leans toward moderate coffee and tea drinking being a small, genuinely helpful piece of long-term brain health, with heavy intake potentially working against you.

The Takeaway Worth Remembering

The next time someone tells you coffee “prevents” Alzheimer’s or Parkinson’s, you’ll know the fuller story.

It’s not a magic shield. It’s a moderate, consistent habit that seems to nudge the odds in your favor, within a fairly specific range.

Two to three cups of real, caffeinated coffee a day looks like a sensible target based on what the science currently shows.

Five, six, or more cups a day might be working against the very outcome you’re hoping for.

Maybe the most interesting part of this research isn’t about coffee at all.

It’s a reminder that almost nothing in health science is simply “good” or “bad.”

Most things are good at the right dose, and that’s a far more useful lesson than any single cup of coffee could ever teach you.
